Renewing Child's Passport (Under Age 16)

Personal Appearance

All children under age 16 must apply in person, accompanied by at least one parent. They cannot apply by mail; no exceptions can be made.

Two-Parent Consent Requirement For Passport Applications

For Minors Under The Age Of 16
All children under the age of 16 are required to appear at the Embassy with both parents for all passport services. If only one parent is able to attend, please see "Application by One Parent" below.

In addition to the requirements listed above, you will be required to submit the following:

Parents' photo identification: Parents accompanying their child to the Embassy must submit their valid U.S. or foreign passports.

Proof of parentage:

Certified U.S. state birth certificate (either vital statistics or Department of Health) with parents' names; or

Certified foreign birth certificate (with parents' names and translation, if necessary); or

Consular Report of Birth Abroad (Form FS-240) (with parents' names); or

Certification of Birth Abroad (Form DS-1350) (with parents' names);

Adoption decree (with adopting parents' names); or

Court Order establishing custody; or

Court Order establishing guardianship.
Note: If your name is not the same as your child's last name or the name listed on your proof of parentage, you must submit documentation that will link your names or link you to the child (marriage/divorce certificate, court order);

Application By One Parent

If only one parent is able to appear, one of the following will be required in addition to all requirements above:

Form DS-3053 (Statement of Consent) completed by the absent parent must be notarized before a U.S. Notary Public. The notary must place his/her official seal upon the document. American Embassies and Consulates abroad can also notarize Form DS-3053 (Statement of Consent).

OR

Evidence of the appearing parent's sole authority to apply for the passport in the form of one of the following:

Consular Report of Birth Abroad (Form FS-240) or Certification of Birth Abroad (Form DS-1350) listing only applying parent; or

Child's birth certificate listing only applying parent; or

Adoption decree (if applying parent is sole adopting parent); or

Court order granting sole custody to the applying parent (i.e. the absent parent has no access to the child. Child's travel must not be restricted by that order); or

Judicial declaration of incompetence of non-applying parent; or

Death certificate of non-applying parent

Exceptions may be made in cases of exigent or special family circumstances as approved by the Department of State. If it is determined that exigent or special family circumstances apply, the applying parent/ guardian must make and sign a statement under the penalty of perjury describing the exigent or special family circumstances.

Application Form

Please complete one copy of Form DS-11, available for download. Forms are also available for pick-up in our offices.

Note that the form you print must be clear and sharp, with black print on white paper, 8 1/2 inches by 11 inches, or A4 size in Pakistan. Thermal paper, dye-sublimation paper, special inkjet paper, and other shiny papers are not acceptable.

Both parents sign their names and the child's name for applicants under age 16. The child does not sign for him/herself. All applicants must be in Pakistan for us to process their application.

Two Photos

The photos must be sized 2" X 2" (or 5 cm X 5 cm) with a white background.

Be careful with the size: "passport photos" you get at a local photographer's shop are likely too small and are intended for Pakistani people to use to obtain a Pakistani passport.

Please don't staple the photos to the application form. Just write the applicant's name on the back of the photos with a felt tip pen (ball point can press through and mar the picture) and include them with the application.

One of the most common problems with passport applications we receive is that the photos are the wrong size. The photo in your current US passport may be a different size from what is needed for a new passport, as standards have changed from time-to-time.

The best way to make sure your photos are the right size is to download and print out a passport application (see above). The drawing of a proper photo is right on the application form.

Age Progression Photographs

If your child’s appearance has changed significantly since his or her last passport was issued, we need a progression of photographs of the child from that time to the present. Photos should be taken no more than six months apart, dated and arranged in chronological order.

Proof of Birth

Please include a copy of one of the following documents:


Certified U.S. birth certificate (with parents' names); or

Consular Report of Birth Abroad (FS-240) (with parents' names); or

Certification of Birth Abroad (DS-1350)(with parents' names); or

Adoption decree (with adopting parents' names); or

Court order establishing custody; or

Court order establishing guardianship

If the parent(s)/guardian current name(s) is/are other than what is stated on these documents, evidence of legal name change (e.g. marriage, divorce, court-ordered name change) is required.

Fee

The fee for a renewal passport is $85 for children. We accept cash, U.S. dollars or Pakistani Ruppees. We cannot accept checks or credit cards at any of our offices in Pakistan.

How Long Does It Take?

Processing time is about three to four weeks after we receive your application in good order, as all passport printing now must be done in the U.S. However, because computers break, and mail is sometimes delivered late, please don't make any unchangeable travel plans until you have your new passport in hand.
