Online Application Form Available For Non-Immigrant Visa Applicants

All non-immigrant visa (NIV) applicants must use the Electronic Visa Application Form (EVAF) format when applying for non-immigrant visas to the US. The EVAF is an online form that is accessible from any internet-connected computer. Applicants complete the EVAF online, print it on a laser-jet quality printer (please do not print the forms with an ink-jet printer as the bar code details can cannot be read/used), and bring it to an American Express/SpeedEx office to pay for their fees and to schedule a visa interview date. Because of the unique barcode that is printed on each EVAF, this new form improves the efficiency and speed of the application process for all applicants.

NOTE: Please check that the large barcode on the third page of the EVAF is completely and clearly printed. If the barcode is broken or only partially printed, your data will not be accepted. Use good quality paper and a recent model laser or inkjet printer for best results. Do not use dot matrix printers. Do not use dashes, slashes or hyphens when you enter data on the form as these characters will prevent your data from being accepted into our system.

What is the difference between the EVAF and the DS-156 form? What is the EVAF?

The EVAF and the DS-156 are exactly the same form. Additional forms, such as the DS-157 or DS-158, should be completed as necessary, in addition to the EVAF/DS-156. The EVAF is a quick, convenient, and easy way to fill out your application and to schedule your visa appointment.

Who should use the EVAF?

All non-immigrant visa applicants must use the EVAF.

What if I don't have a computer or what if I don't have internet access?

If you can't complete the EVAF online, you may handwrite/type the DS-156 and take it to the nearest American Express/Speed Ex office in Pakistan where your data will be entered into our system and your appointment will be scheduled. However, it may take a little longer before you receive a visa interview date. You can even complete the form at an internet café, since the EVAF can be used on any computer with an internet connection.

If I fill out the EVAF, do I still have to visit an American Express or SpeedEx office? Or can I send the form by email?

At this time, you still need to visit an office in person in order to pay your $131USD visa application fee and to schedule your appointment.

Is the EVAF a secure document?

Yes. The EVAF has been used by millions of applicants since 2003 and your data is stored securely in the State Department's visa database.

Is there a fee for using the EVAF?

No. The EVAF is free; all you need are a computer with an internet connection and a laser-jet quality printer. However, EVAF users are still required to pay the $131USD visa application fee.

What if I make a mistake on the EVAF?

If you notice a mistake after you've scheduled your appointment, just make corrections to the form by hand, and the information will be updated in the database.
